STEP 1

Determine what class of medical certification you need.

STEP 2

Consider whether you are ready to schedule an FAA MEDICAL CERTIFCATION EXAM. If this is your first exam and you are on medications or have significant medical history, consider scheduling a CONSULTATION first to go over potential requirements that might be important for you to understand.

Fee Structure

FAA medical exam: $150. All classes

Consultation: $50. Consider if you have medications or medical issues that might require additional information from your treating physician(s) to submit to the FAA. If you wait until your FAA medical exam to discuss and have to collect more paperwork, there may not be enough time for you to get the required information and the FAA will deny your application.

EKG: $50. Required only for FIRST CLASS applicants at age 35, then annually at age 40.

Color Vision Testing: $50. Required for first time medical exams ONLY. If you have passed color vision for a prior exam, you do NOT have to be retested. We use the FAA-approved Waggoner CCVT Color Test which is administered on an iPad.

We accept credit/debit cards or cash only. No checks.


Tip 1

Contact a third-party source, such as AOPA, if you have any concerns about your current health and/or medical history.

Tip 2

When in doubt, ask questions — to either the AOPA, call the FAA directly, or reach out to your AME (Aviation Medical Examiner.)

Tip 3

Be sure to honestly and thoroughly answer the questions on your FAA MedXPress application. Note that it asks for ANY medical issues in your lifetime, not just recent ones. Answer truthfully about any history of convictions/legal issues, no matter how long ago. If you have reported an issue on a prior MedXPress application, you should still list it on subsequent applications, but then note “PRNC” in the comments which means “previously reported, no changes.” In signing the MedXPress application you are legally attesting that your application is accurate

Tip 4

Details! Make sure you fill in your name the same way you do in other places – such as on the FAA student pilot application. For example, if you put “Jr.” on your medical form, but not on your student pilot application, it could cause a delay.

Todd Page, MD, graduated from the University of Colorado with a bachelor’s degree in music and then went to Indiana University for medical school, followed by a family medicine residency in South Bend, Indiana. He’s practiced as a family physician for over 25 years in the Fort Wayne area after moving to Indiana from Colorado.

Beth Berghoff, RN, brings more than 25 years of nursing experience to our clinic. She has extensive experience working in pediatrics, obstetrics and gynecology, and medical research. She grew up in the Fort Wayne area.

The Sweetwater Medical Clinic is located inside the Sweetwater Sound campus as a service and convenience to its employees.

The doctor on staff (Dr. Page) offers aviation medical exams out of the same office.

To get to the Sweetwater Medical Clinic, come in the main Sweetwater entrance on the north side of the building. If you are unsure of where the clinic is once inside, simply ask our concierge team at the entrance and they’ll make sure you find us!
